PDC Form
You may
access the PDC form through CHIEF 98. Once you have accessed
CHIEF 98 through AppleShare, copy the PDC Form to your hard
drive. Open Appendix C - Individual Plan and use it however
you see fit.
We are also working on forms you can download
from the internet. They are currently in development, so if
they don't work as you think they should, let
